COVID-19 Self-Assessment Tool

At TELUS Health, our customers and our team are our top priority. As part of our efforts to safeguard the health and safety of our team members and customers, we’ve launched a COVID-19 self-assessment tool. Developed by medical professionals, public health experts and infectious disease specialists, the tool follows Public Health guidelines and helps Canadians assess their condition in the comfort of their own home, empowering them with information to make better health decisions. Guidelines are changing on a daily basis, so check back often.

Your answers are anonymous but may be aggregated to help healthcare providers gauge the spread of COVID-19 in your province.

Do you have a fever, chills or shakes?


Do you have a new or worsening cough?


Are you experiencing any of the following symptoms:

  • severe difficulty breathing (for example, struggling for each breath, speaking in single words),
  • severe chest pain,
  • having a very hard time waking up,
  • feeling confused, or
  • lost consciousness?


Are you 60 years of age or older?


Do you have any of the following medical conditions:

  • diabetes,
  • heart disease,
  • active cancer,
  • history of stroke,
  • asthma,
  • COPD, or
  • a condition requiring dialysis?


Have you traveled outside of Canada within the last 14 days?


Have you had close contact with someone who is coughing, has a fever, or is otherwise sick and has been outside of Canada in the last 14 days?
